The following is a list of some of the possible sources of verification that can be used to demonstrate whether certain technical eligibility factors are met. For further discussion of all eligibility factors and their separate special verification requirements, refer to the program manual.

When verification of age is needed, the following are some of the acceptable sources of verification:

The recipient should be the primary source of verifying documents. In unusual cases in which the recipient cannot provide such documents, the worker may use SFN 419, "Request to Division of Vital Statistics for Confidential Verification of Birth," to request a North Dakota or out-of-state record of birth from the State Department of Health. If an out-of-state record of birth is requested from the North Dakota State Health Department, Division of Vital Records, the recipient will be responsible for the fee to acquire the record. The fees vary from state to state; contact the Division of Vital Records for information on individual state fees.
In rare cases in which a person's birth year can be established, but the precise day of birth cannot be established by documents or by a responsible collateral contact, the date of birth shall be established as July 1.
