A complaint form is incorporated in the brochure, "Your Civil Rights Under North Dakota Department of Human Services Programs". Complaints of discrimination may be filed by clients of programs and services of the Department of Human Services by completing that form, or in the alternative, the client, patient or recipient of services may complete SFN 143.
