This form is to be completed by a recipient who is attending postsecondary education. The form is to aid the worker in determining if the client is eligible for postsecondary education and if the postsecondary education is an allowable education/training for which child care will be covered.
This form is available through the Department of Human Services and may also be obtained electronically via E-Forms.
